Please see below for ia letter form the pharmacy service manager at Leicester Royal Infirmary giving notification to community pharmacies to expect to see FP10 prescriptions coming out of the LRI Ophthamology department for the forseeable future.
These prescriptions will have hospital details on them(instead of GP practice details).
Please note the hospital pharmacist contact details (for prescriber queries) and the helpful formulary which can be downloaded for your reference.
