How can capacity be increased within Primary Care?

Through meeting with and talking to community pharmacists.  Service resdesign should look at the patient pathway and the added value that can be gained at each point that a patient meets with a clinician.  Therefore all primary care personnel should be actively talking to any person that may be able to impact upon the patient's care.

The GPs are forming consortia within the PCT to work together in commissioning groups.  Any plans submitted under PBC should be a coordinated approach with your local consortium.  The groups or GPs, the clinical lead, email addressess and PCT contact are attached in the following links and should be the first port of call.
