PS-AS-1 Premises    (PDF format)
This form is used to register the premises to be used for Advanced Services.

AS1Pharmacist Listing   (PDF format)
This form is used to register the pharmacist who is accredited to carry out Advanced Services.

PS-AS-1 Claim     (PDF format)
This form is used for submission by the contractor to claim for Advanced Services carried out.

MUR pharmacists guidance   (PDF format)

MUR hard copy triplicate pads that are in circulation only bear the English NHS logo. Contractors in Wales can use these.
