New Medicines Service
New Medicines Service - Pharmacy Contractor Declaration Form - a completed copy of this form should be submitted by the pharmacy contractor to their PCT prior to the provision of the New Medicines Service (NMS). The PCT does not need to acknowledge receipt of the form before th pharmacy commences provision of the service.
